What explains recklessness at work?

Irresponsible behavior is typically a one-off for example running a red light unlike consistently running red lights which is reckless. It is human to make mistakes but recklessness can be said to be a vice where the person always has a complete disregard for themselves and/or others. One easy one at work is when someone is known to look to argue or pick fights with with those who they don’t like, you know who they are. So, how hard or not is it to deal with recklessness?
